Please have your CareCard or current medical coverage and requisition for identification when you arrive at the laboratory. Check that the requistion includes your full legal name, date of birth and Care Card Number.All tests must be ordered by a physician.
For most tests you can simply show up during laboratory outpatient hours. A few tests require special arrangements.
